The information potentially exposed in the data breach varies from person to person but may include names, dates of birth, medical testing information, laboratory test results, health insurance information, and, for a limited subset of patients, Social Security numbers. Information belonging to current and former employees may also have been exposed, including Social Security numbers, government-issued identification numbers, and financial account information.
According to information reported to the U.S. Department of Health and Human Services (HHS), the data breach affected approximately 2.8 million individuals.
